To begin, this application requires completion by both the child's caregiver and the child. Initially, we will gather your contact information. Following this, we will need specific details about the prospective athlete.

Please note that part of this application pertains to the athlete's incarcerated parent. It's crucial to provide all requested information accurately to ensure your child's eligibility.

The final section comprises questions designed for the child. While the child should complete these independently, your assistance is permissible if necessary.
